How Ohio Nursing Homes Are Rated

Every Nursing Home in Ohio is rated by Medicare on a 5 star system. Each nursing home gets an Overall rating, a Health Inspection rating, a rating for Staffing and a rating for Quality Measures. These ratings can be seen at the Medicare website here.

When a nursing home gets a serious health or fire safety citation, or fails to correct a citation for a long period of time, this can result in a penalty. A penalty can be a fine against the nursing home or a denied payment from Medicare. There are federal penalties and state penalties.

This means that the nursing home has been cited for abuse.

CMS also keeps track of nursing homes that have a history of serious quality issues and are included in a special program to stimulate improvements in their quality of care. This is known as the Special Focus Facility (SFF) program.

How Are These Bad Nursing Homes Identified?

The Centers for Medicare & Medicaid Services (CMS) and State Agencies (SAs) inspect nursing homes on a regular basis to determine if they are providing the quality of care that Medicare and Medicaid require in order to protect and improve residents’ health and safety. When nursing homes do not meet CMS’ health care or fire safety standards, these instances are cited as deficiencies, and CMS requires that the problems be corrected. Most nursing homes have some deficiencies, with the average being six to seven deficiencies per standard health inspection.

However, CMS has found that a minority of nursing homes have:

  • More problems than other nursing homes (about twice the average number of deficiencies);
  • More serious problems than most other nursing homes (including harm or injury experienced by residents);
  • A pattern of serious problems that have persisted over a long period of time (as measured over approximately three years before the date the nursing home was first put on the SFF list).

Although such nursing homes may periodically institute enough improvements to correct problems identified on one inspection, significant problems would often resurface by the time of the next inspection. Such facilities with a “yo-yo” compliance history rarely address underlying systemic problems that give rise to repeated cycles of serious deficiencies, which pose risks to residents’ health and safety. To address this problem CMS operates the SFF program. Many nursing homes on this list are part of the Special Facility Program. Others may not be officially part of the SFF program but they have been flagged as having significant issues.
